Supporting Recovery Beyond Discharge: Insights from MHO’s Reflections Behavioral Health Companion App

At MHO, we understand recovery doesn’t end when treatment does. That’s why we developed the Reflections Behavioral Health Companion app—a tool designed to help patients stay engaged in their behavioral health journey and give providers valuable insights into long-term outcomes of their patients in aggregate.

Through the app, patients can:

  • Answer questions about aftercare participation
  • Complete clinical outcomes assessments used during treatment to monitor ongoing symptoms
  • Check in daily to track mental health and medication adherence
  • Journal their thoughts and experiences

To date, patients have contributed over 1,700 data points, offering a rich foundation for understanding post-discharge experiences. These early insights are shared here through a series of case studies, highlighting common patterns and themes in recovery. In these case studies, patients completed both daily check-ins and clinical assessments, with some also contributing data during treatment—allowing for a more complete view of their recovery journey.

What Do These Data Mean for Patients and Providers?

Our initial analysis reveals that while each patient’s path is unique, there are consistent indicators that may signal risk of readmission. These include:

  • Skipping appointments
  • Medication non-compliance
  • Sudden increases in symptom severity

Even in this early stage, data show strong potential to support both patients and providers in meaningful ways.

For Providers:

  • Identify traits of patients who maintain progress—or those who may need additional support
  • Predict patterns linked to non-compliance or readmission
  • Share evidence of long-term success with stakeholders
  • Reinforce the importance of post-discharge plans with patients
  • Estimate readmission risk using self-reported data
  • Track patient journeys across multiple programs

For Patients:

  • Build self-awareness and stay engaged in recovery
  • Monitor mental health trends over time
  • Share data with future providers for more personalized care
  • Cues for medication and aftercare tasks
  • Get early alerts when symptoms begin to change

 

Case studies, like the ones shared here, focus on a single patient’s experience. These are valuable for early data exploration and illustrating how this data may help providers and patients. They offer a close-up view of how data can be applied in real-world scenarios. In contrast, aggregate reporting—as will soon be available for providers in Reflections—presents data from many patients combined. This approach helps identify larger trends, measure program effectiveness, and guide strategic improvements to care.
